Could be getting nipped or could be disease. What else is in there with him?
I found melafix helpful for fin rot on a betta when he got sick after I forgot to turn his heater back on after cleaning. (I felt terrible!)
Be sure you know what you're dealing with before adding any meds to the tank, though. Don't just start throwing chemicals in there willy-nilly.

With it that bad you just need to focus on getting that pleco back to it's healthy self google "myfishkeeper" and get this asap. I used it on my peacock bass and the fin's were twice as bad. A couple cap's of this puppy he was kicking like a mule and looking like a sexy bass instead of one that look's like it got stomped out and thrown back in the tank
